This form tells you:

• Terms and conditions of booking

• What you should do before and after an acupuncture treatment

• A list of possible side effects and what you should do in the unfortunate incidence of anything happening.

This form is a checklist of what me, your acupuncturist, needs to know before my arrival.  Examples are:

• Access to your property and room where you would like treatment

• Parking

• Access to a sink

• Any pets or people who will be in the home at the time of your treatment

This form is a screening tool for signs and symptoms of COVID.  It is also a disclaimer that I will be on your premises and in contact with you for at least one hour and you are aware of potential transmission.

Sometimes I will ask you to continue treatment with moxa.  I will thoroughly explain and show you how to use it.  This form will be a chance for us to both feel confident in usage and signed accordingly.
